HESI A2 Critical Thinking Section: What Nursing Applicants Must Know

The HESI A2 Critical Thinking section tests clinical judgment across 4 core domains: Problem Solving, Decision Making, Meaningful Reasoning, and Argument Analysis. Questions present real-world patient scenarios where you must prioritize care using Maslow's Hierarchy of Needs and ABCs (Airway, Breathing, Circulation).
The HESI A2 Critical Thinking section is one of the most unpredictable modules required by top nursing schools. Unlike vocabulary or anatomy, you cannot pass by pure memorization — you must reason like a nurse.
Many pre-nursing students score 90%+ on math and reading only to be caught off guard by critical thinking scenarios. If your nursing program requires this section (such as Chamberlain University, UT Health, or Grand Canyon University), mastering clinical prioritization framework is mandatory.
The 4 Core Domains Tested on HESI Critical Thinking
The HESI Critical Thinking module consists of 25 to 30 scenario-based questions. The exam evaluates your decision-making abilities across four distinct domains:
- 1. Problem Solving: Identifying the root cause of a clinical emergency or patient decline.
- 2. Decision Making: Choosing the immediate first action a nurse must take before delegating or notifying a physician.
- 3. Meaningful Reasoning: Differentiating between subjective patient statements and objective assessment data.
- 4. Argument Analysis: Evaluating conflicting clinical information or competing patient needs.
Always apply the ABCs (Airway, Breathing, Circulation) first, followed by Maslow's Hierarchy of Needs (physiological needs before safety or emotional needs). If a patient cannot breathe, no other nursing intervention matters!

Question:
A nurse enters a patient room and observes the patient gasping for air, clutching their chest, and exhibiting a bluish tint around the lips. What is the nurse's immediate first action?
